Non-glycemic-dependent reduction of late pregnancy A1C levels in women with type 1 diabetes.

چکیده
A reduction of A1C levels during pregnancy in women without diabetes has been reported (1–6), suggesting that the goal for A1C during pregnancy complicated with diabetes should be lowered. The decrease in fasting blood glucose levels during the first trimester of normal pregnancy (7,8) may explain the lower A1C levels found in early pregnancy (1,2,3,5). A further decrease of A1C in late pregnancy in women without diabetes has been reported by Nielsen et al. (3), which may be related to decreased erythrocyte life span (9,10). In women with type 1 diabetes, strict glycemic control during pregnancy leads to a reduction in A1C levels, which may be insufficient to prevent the complications of pregnancy in type 1 diabetes (11,12). Since the decrease in A1C during normal pregnancy is related to lower blood glucose and to decreased erythrocyte lifespan (6 –10), some of the reduction of A1C observed in pregnant women with type 1 diabetes will not be related to improved glycemic control. Therefore, A1C levels in pregnancies complicated by diabetes can be misleading when evaluating the degree of glycemic control. The aim of this study was to quantify the decrease in A1C levels during late pregnancy in women with type 1 diabetes who were not dependent on glycemic control.

Optimal Glycemic Control, Pre-eclampsia, and Gestational Hypertension in Women With Type 1 Diabetes in the Diabetes and Pre-eclampsia Intervention Trial
OBJECTIVE To assess the relationship between glycemic control, pre-eclampsia, and gestational hypertension in women with type 1 diabetes. RESEARCH DESIGN AND METHODS Pregnancy outcome (pre-eclampsia or gestational hypertension) was assessed prospectively in 749 women from the randomized controlled Diabetes and Pre-eclampsia Intervention Trial (DAPIT). HbA(1c) (A1C) values were available up to...
متن کاملA1C but Not Serum Glycated Albumin Is Elevated Because of Iron Deficiency in Late Pregnancy in Diabetic Women
OBJECTIVE We have already reported that A1C is elevated because of iron deficiency in late pregnancy among nondiabetic pregnant women. This report examined whether the same phenomenon is observed in pregnant women with diabetes. RESEARCH DESIGN AND METHODS This longitudinal study was conducted in 17 pregnant women with diabetes (20-35 weeks of pregnancy). A1C, serum glycated albumin, erythrocyt...
متن کاملPostpregnancy Glycemic Control and Weight Changes in Type 1 Diabetic Women
OBJECTIVE Pregnancy in type 1 diabetes requires excellent glycemic control. Most pregnant type 1 diabetic women achieve normoglycemia; however, there is scarce data on their postdelivery characteristics. We aimed to examine postpregnancy glycemic control and weight changes in type 1 diabetes. RESEARCH DESIGN AND METHODS We identified and followed (median 20 months) 254 women with singleton pr...
متن کاملManagement of Type 1 Diabetes in Pregnancy
Women with type 1 diabetes (T1DM) have unique needs during the preconception, pregnancy, and postpartum periods. Preconception counseling is essential for women with T1DM to minimize pregnancy risks. The goals of preconception care should be tight glycemic control with a hemoglobin A1c (A1C) < 7 % and as close to 6 % as possible, without significant hypoglycemia. This will lower risks of congen...
متن کاملAssociated factors of poor glycemic control in Iranian diabetic patients
Introduction: According to the world health organization the prevalence of type 2 diabetes in our country and world will increase sharply by 2030. Because improving glycemic control delays the onset and progression of diabetes complications, recognizing related factors is key step in the effective treatment of these patients. Therefore, this study was performed to determine the predictors of po...
